Crew action caused loss of control and stalling of AirAsia plane
Human error caused the crash of the AirAsia plane in the Java Sea a year ago, killing all 162 people on board, Indonesian investigators said this morning.
A fault with the rudder control system on the Airbus A320 was a major factor in the crash. The system had a cracked joint that constantly malfunctioned, including four times during the flight – and 23 times during the year.
This led to the pilots disengaging the autopilot in stormy weather to try to fix the situation, but they then lost control of the aircraft.
The plane crashed less than halfway into a two-hour flight from Surabaya in Indonesia to Singapore on December 28. Indonesia’s official National Transportation Safety Committee led the investigations.
