Senior executive vice president bows to apologise to TV cameras

Even in the high-tech world of the airline industry, human error can still bring disaster. A catastrophe was narrowly avoided, an investigation revealed this week, when an All Nippon Airways aircraft almost flipped over mid-flight on September 6. The incident occurred when a co-pilot pushed a wrong button while attempting to open the cockpit door for the captain, who was returning from the toilet.
Awkwardly for ANA, news of the investigation was released only several hours after the first Boeing 787 Dreamliner finally touched down in Tokyo. At a news conference, Shin Nagase, a senior executive vice president at ANA, bowed before TV cameras to apologise for the incident.
